doctor” information.
After-hours Procedures
When you call our office after-hours you
will hear a message guiding you through a menu
that can help you access the extension number
that would most likely meet your needs. If
you know the extension you want to reach, just
enter it at any time during the message or
enter “0” to reach the Operator.
Our nursing staff screens most telephone calls,
and special problems are handled by consultation
with the providers.
Telephone calls received after 4:30 p.m. will
be returned the next “regular” business
day, unless they are urgent.

The email is checked frequently, but should not be used for emergencies
or medical advice.

at Mercy Medical Center is (970) 764-3140.
It is staffed 24 hours a day, seven days
a week, 365 days a year to answer your questions
or if you think you are in labor.

We accept most
insurance plans and participate in selected managed
care programs. We are not a primary care office,
so are not designated as primary care providers
by the HMO plans
Please remember that your specific plan may
not cover all the services that you may have
received. Some services may not be deemed as “medically
necessary”. Some services may be applied
to your deductible. As is the customary practice
with many plans, a co-pay is collected at the
time of an office visit. For your convenience
Visa, Discover, and Mastercard are accepted.
Disability Forms
Our office will fill out one disability form
per medical issue, including pregnancy, free
of charge. A nominal fee will be charged to
fill out any other form(s).
Medical Records
Release of information contained in your medical
records requires your written permission. There
is a fee for copying medical records.
